Your enrollment fee will be required at the time of application and to guarantee your space in the practice. Once accepted, Membership payments are payable on the 1st of each month.  

 

If you are anMedicare or HMO patient please call us as there are important details that we need to discuss regarding membership.

This is not a replacement to Insurance.

We are out of network with all commercial insurance plans and will not bill insurance or collect co-pays. We will continue to accept Medicare and bill them for covered services. Medicare patients are still required to pay membership fees in order to remain part of the practice.
